We collect, use, and disclose your personal health information with your consent, which may be express or implied within your “circle of care.” You may withhold or withdraw consent — subject to legal and professional limits — by contacting us, though doing so may affect our ability to provide certain care.
How We Protect It
We use administrative, technical, and physical safeguards to keep your personal health information secure, and we limit access to those who need it to provide your care or operate our practice.
How We Disclose It
We do not disclose your personal health information except as permitted or required by law — for example, to other health providers involved in your care, to your insurer with your consent, or where required by a regulatory body or court.
